CUISTODIAN DECLARATION
- For parents: Normal ID requirements
- Custodian needs to bring an ID to confirm that the custodian is a Canadian citizen or permanent resident (e.g. Canadian passport, citizenship certificate, Nexus, PR card, Canadian birth certificate)
- If the student lives with the custodian but the custodian’s ID does not have custodian’s current address, custodian has to bring other document that confirms the custodian’s current address
